What is the School Authorization Form?
The School Authorization and Instructional Role Form is designed to facilitate agreements between Relay and your PK-12 school so that both parties may best support your development as a teacher. The form also supports state-specific requirements of teacher preparation programs, including requirements regarding mentor teachers. This form is completed in two parts.

Part 1: You complete the Student portion, including your school leader's name and email address. When you click submit, an email will be sent to your school leader with a link for them to complete their portion.

Part 2: Your school leader then completes their section, verifying your school placement and acknowledging school responsibilities.
Note: If you have not yet secured a specific instructional position or do not know who your school leader will be, you'll complete this form once you have a position.
Deadlines:
For students enrolling in the spring 2026 session:

School leader portion due by Friday, February 6, 2026
Failure to submit both the student and school leader portions of the School Authorization and Instructional Role form will result in administrative withdrawal from Relay.
Who should I list as my School Leader?
You should list your Principal, Assistant Principal, or HR representative as your School Leader.
Where can I find the link to my portion of the form?
The School Authorization and Instructional Role Form will be sent to your Relay school email, along with instructions on how to access it through your My Relay Portal. If you do not see this email, please contact Lendora Johnson at [email protected], and she will resend the email to you. You can also follow these steps:

Navigate to My Profile

Open the School Authorization and Instructional Role form and fill in the required information.
How do I re-send the link to my school leader?

Email [email protected] and request that the email be resent to your school leader. Be sure to include the name and email of the school leader you submitted on your student form.
How do I change the name, school, or contact information for my school leader?
If you need to make changes to your School Authorization and Instructional Role Form, return to My Relay Portal, navigate to My Profile, open the form, update the information, and resubmit it.
