The Relay Support module includes access to contact the Relay Support Team or view the Relay Support Center and direct links to the most commonly requested student forms. Note - you can access these materials through the Relay Support Center as well!
Student Record Form
- If you would like to request a change to the information Relay has on file for you, such as preferred name, address, teacher preparation program participation, or teaching placement location.
Area of Study Update Form
- If you would like to request a change to your area of study (AOS). Remember that changing your area of study may impact certification, enrollment, and tuition.
Withdraw/LOA Request
- If you intend to withdraw or take a leave of absence from Relay, submit this request so that the Registrar’s Office can support you through this process.
Transcript Orders
You can submit an order for an official transcript here. Transcript orders are processed by the Registrar’s Office and can take anywhere from one to three weeks to be processed. Unofficial transcripts can be printed from the Relay Student Portal.
Disability Disclosure Form
- Please only complete this form if you want to log your disability/disorder with the Office of Student Affairs and/or you are requesting accommodations for the current Academic School year. If you have any questions when completing the form, please email [email protected].
Directory Non-Disclosure Form
- If you are a student who does not want any or all directory information to be disclosed without prior consent.
Enrollment Verification Form
- If you need to request enrollment verification.
Re-enrollment/Transfer Application Form
- Students previously admitted to and enrolled at Relay but who took a leave of absence or withdrew can submit the reenrollment/transfer application to Relay. Students should plan to apply two months prior to the start of the requested reentrance term and no later than the requested reentrance term census date.
Ombudsman Service Request Form
- The Relay Student Ombudsman Service provides a safe space for students to confidentially discuss institutional and academic concerns, clarification of policies, and other challenges that impact academic progress.
Student Grievance Form
- Relay provides to its students a student grievance policy so that students have a process for seeking to resolve alleged violations, of a written Relay policy, that directly affect students, where the violation is caused by any member of the Relay community while acting in an official capacity (e.g., faculty member, administrator, staff member). Students have the right to file a grievance without fear of unfair treatment from Relay faculty or staff.
